Annotation: This final report describes a 2005-2010 project to incorporate a family resource center into an existing pediatric practice to offer family and community focused health care for low-income children and their families. The project included providing bilingual services in a culturally competent manner, assisting families with referrals for basic needs, developing a local resource manual, working as a liaison with Medicaid and state insurance programs to ensure eligibility, and providing families with transportation services to attend medical needs. Report contents include a description of the project, its goals and objectives, methodology, evaluation, results and outcomes, and dissemination and utilization of results. Annotation: This report describes the activities and outcomes of a 2001-2005 Healthy Start project in five counties of rural Missouri to enhance the community's perinatal service system and to reduce infant mortality and racial disparities in infant health. Report contents include an overview of racial and ethnic disparities in the area served by the project and project implementation including outreach, client recruitment and case management, interconceptional care, depression screening and referral, health education and training, local health system action plans, consortium activities, and collaboration and coordination. Annotation: This summary and case study describe a pilot project by Molina Healthcare of Michigan to improve Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) rates and early childhood developmental screening at 11 provider practices using the Best Clinical and Administrative Practices (BCAP) framework. Sample materials from the project, called Baby Steps Toward Health, include outreach materials on well-child visits for parents and provider education materials about EPSDT components, proper documentation, coding, and referral sources for children with developmental delays. A chart outlining the Medicaid well-child check-ups timeline: birth to 21 is also provided.

Annotation: This report discusses the importance of prenatal care and explores the impact of the Medicaid eligibility expansions and systems improvements on early prenatal coverage and access to care in California over the past two decades. Chapter topics include an overview and background of the importance of obtaining prenatal care, trends of prenatal care use and coverage in California during 1980-1999, a survey of California's maternity population, improving early access to Medicaid coverage in California, a review of non-insurance barriers to early prenatal care, and conclusions and policy recommendations for improving access to prenatal care. A reference list and technical appendix providing a general description of the data sources, study samples, variables, and analysis plans used are provided. Statistical data are provided in chart formats throughout the report.

Annotation: This toolkit begins with a brief discussion of the process improvement model used in Best Clinical and Administrative Practices (BCAP). It then presents the "typology for improvement" developed for the Toward Improving Birth Outcomes workgroup, followed by a separate chapter for each typology category. For each of these categories, an inventory of change strategies is listed, followed by case studies of pilot projects. The last chapter outlines effective communication tactics to facilitate change. The appendices provide sample tools from BCAP workgroup health plans and other relevant materials, including program forms, risk assessment forms, pilot program materials; recommended readings, and the BCAP network and contacts.
