Babyscripts

Annotation: Babyscripts is a patient-centered maternal health program that provides risk detection and virtual care support throughout pregnancy and postpartum. The program offers a comprehensive remote patient monitoring system designed specifically for obstetrics to improve maternal health outcomes. The program includes three main components: perinatal education with clinically-validated resources accessible through a mobile app; remote blood pressure monitoring to enable real-time risk management and intervention; and mental health assessments to identify risk and support needs. Founded in 2014, The program is designed for health systems, independent practices, and health plans/payers, offering tools that have demonstrated measurable outcomes, including a 13-day reduction in time to detect preeclampsia and 93% at-home blood pressure ascertainment for both White and Black patients. The platform emphasizes increasing patient engagement while focusing on equity and accessibility through diverse visual imagery, accessible content, and equitable digital tools.

Cayaba Care

Annotation: Cayaba Care is a community-based maternity program that provides wrap-around support throughout pregnancy and postpartum to improve maternal health outcomes. The program offers personalized maternal healthcare support through Maternity Navigators who are community health workers and certified doulas working closely with members both in-home and virtually. Center for Black Women's Wellness

Annotation: The Center for Black Women's Wellness (CBWW) is a nonprofit organization dedicated to improving the health and well-being of underserved Black women and their families. Minnesota Perinatal Quality Collaborative (MNPQC)

Annotation: The Minnesota Perinatal Quality Collaborative (MNPQC) seeks to improve maternal and infant health outcomes with emphasis on racial and ethnic health inequities. The MNPQC is a network of organizations, medical providers, content experts and community voices led by the Minnesota Perinatal Organization (MPO) in partnership with the Minnesota Department of Health (MDH). University of North Carolina, Collaborative for Maternal and Infant Health

Annotation: UNC Collaborative for Maternal and Infant Health (CMIH) works to improve the health and well-being of women and families across North Carolina through research, community partnerships, and health care innovations. CMIH 1) collaborates with North Carolina hospitals, clinics, and communities to provide training on maternal warning signs and assistance with planning and implementing obstetric emergency protocols; 2) provides quality perinatal care to women across North Carolina; 3) provides clinicians with patient-education materials, including information on home blood pressure monitoring, post-birth warning signs, and breastfeeding; and 4) works with partners across the state to develop materials and trainings to best address care for pregnant and postpartum in NC jails and prisons. The collaborative also produces and disseminates postpartum self-care resources for new mothers, resources and information about infant safe sleep; and tobacco screening and cessation counseling tools for practitioners and patients.
