211 LA County

Annotation: 211 LA County provides access to a comprehensive range of human services to the people of Los Angeles County, California. It also provides access to the Taxonomy of Human Services, a classification system for indexing resources in information and referral databases that was developed by INFO LINE of Los Angeles, the predecessor to 211 LA County, and that is endorsed by the national association Alliance for Information and Referral Systems and United Way of America as the standard indexing system for the industry.

City of Milwaukee Health Department

First 5 LA

Annotation: First 5 LA is a child-advocacy organization created by California voters to invest tobacco tax revenues in programs for improving the lives of children in Los Angeles County, from prenatal through age 5. The vision is to create a future throughout Los Angeles' communities where all young children are born healthy and raised in a loving and nurturing environment so that they grow up healthy, are eager to learn, and reach their full potential. Programs include Best Start LA, community-developed initiatives; early developmental screening and intervention; family literacy; a parent hotline; Healthy Births; Healthy Kids; oral health and nutrition; partnerships for families; school readiness, universal preschool, and workforce development.

Healthy Baby Network

Annotation: the Healthy Baby Network is a service of Healthy Start Rochester (NY). The network provides individualized case management for vulnerable mothers during prenatal care; walks the streets and providie access directly to those in need; puts ogether a taskforce of mothers, community partners, and 35 different agencies to mobilize our community around perinatal health; and workisdirectly with mothers and families to provide access to care, job training, and housing security.

Healthy Programs in Michigan

Annotation: This group promotes the evidence-based self-management and physical activity programs that are available in Michigan communities and to support the partnerships that are active in their implementation. The programs featured have met the evidence-based criteria of Centers for Disease Control and Prevention Arthritis Program and Administration on Aging. Information about partnerships and programs, including class and workshop schedules, leader training, promotional materials, and more are available from their website.

Megaphone Project

Annotation: The Megaphone Project amplifies the voices of under-served communities and those who advocate for them in order to increase political access, inspire action and create social change. The project accomplishes this through message development, media production, and targeted media distribution. Their messages and media productions have contributed to reform in the criminal justice system, public housing, predatory lending, real estate development, public transit and day labor. The Web site provides film clips, access to a mailing list for project announcements, and a store for purchase of DVDs. Its focus is first on the city of Baltimore then on the state of Maryland.

National Association of County and City Health Officials (NACCHO)

Annotation: The National Association of County and City Health Officials (NACCHO) is a national nonprofit organization representing local health departments. NACCHO promotes national policy, develops resources and programs, seeks health equity, and supports effective local public health practice and systems. Program areas include community health, environmental health, public health infrastructure and systems, and public health preparedness. Northern Virginia Health Foundation

Annotation: The Northern Virginia Health Foundation makes strategic grants to nonprofit organizations that address the health care needs of residents of Northern Virginia, with particular emphasis on those who live in Alexandria City, Arlington County, Fairfax City and Fairfax County. Since its inception, the Foundation has awarded grants to organizations that increase access to primary care, mental health, oral/dental health, and/or specialty care for low income uninsured or underinsured residents of Northern Virginia.

Smart Start

Annotation: Smart Start is a childhood initiative in the state of North Carolina designed to help ensure that young children enter school healthy and ready to succeed. This public-private initiative provides early education funding to all of the state's 100 counties through local nonprofit organizations. Smart Start works to improve the quality of child care, make child care more affordable and accessible, and provide access to health services and other family support, Smart Start's National Technical Assistance Center provides technical assistance to communities and states and offers a variety of publications, in both English and Spanish, describing start-up efforts, lessons learned, effective practices, and quality improvement.
