Two appendices provide a summary of the regional meetings that describes the dominant themes and comments made by attendees, as well as presentations.

Annotation: This document describes an action plan for improving the oral health of New Jersey’s infants and young children (from birth through age 6) with emphasis on those who are minorities or from families with low incomes. Topics include a review of the status of oral health in infants and young children and a summary of the pediatric oral health forum, held in May 2007, and its goals and recommendations. Action plan contents include addressing challenges and recommendations in (1) access to care, (2) education, (3) preventive services, (4) state leadership and advocacy, and (5) administrative structures to monitor the quality of services.

Annotation: This report presents findings from a needs assessment survey completed by Head Start and migrant and seasonal Head Start workers related to ten areas determined to be a top priority: health care, services for children experiencing homelessness, family/child assistance, child care, family literacy services, children with disabilities and their families, community services, partnerships with local education agencies, Head Start transition and alignment with K-12, and professional development. Individual sections addresses the levels of cooperation, coordination, or collaboration between the Head Start programs and service providers. The report includes an overview of Head Start programs in Alabama; a review of trends and their implications; and final recommendations. The report was used to provide direction for the development of a required strategic plan that will guide the Alabama Head Start State Collaboration Office to support Head Start grantees in meeting requirements of the Head Start Act.

Annotation: This strategic plan incorporates recommendations for addressing racial and ethnic disparitiies in low birth weight, and is a revision of the state of Connecticut's original strategic plan prepared in May 2008. The revised plan examines risk factors for low birth weight; examines racial and ethnic disparities; and provides recommendations to address disparities in low birth weight. The plan highlights evidence-based interventions and describes planned activities within Connecticut's Family Health Section to address the multiple health determinants and disparities associated with low birth weight in the state.

Annotation: This report provides a detailed plan of action to reduce the burden of asthma in the state of Rhode Island. Included are descriptions of the state's goals and objectives to help improve asthma control among those in Rhode Island with asthma, reduce asthma-related hospitalizations and emergency department visits, and reduce disparities among priority populations (identified using asthma prevalence and hospitalization data). The purpose, mission, and framework of the state plan are explained in detail, and a logic model of the asthma state plan is provided at the end of the report.

The plan, which includes an executive summary, defines infant mental health, summarizes the research; and outlines accomplishments, changes and challenges. Eight goals of the plan are detailed and a chart highlighting service arrays is included.

Annotation: This report discusses the importance of preconception health and describes North Carolina's strategic plan to help ensure that women in the state receive quality preconception care. The report describes how the 5-year preconception health strategic plan will improve the health of women and babies, address health disparities, increase access to care, and decrease the financial burden to families and taxpayers as a result of preterm births and birth defects (the leading causes of infant mortality in North Carolina). The report explains how the strategic plan was developed and discusses its framework, goals, and strategies. It also describes state initiatives to improve women's health during their childbearing years and discusses the history of preconception health in North Carolina. The appendix provides both quantitative and qualitative data on women of reproductive age who reside in the state.

Annotation: This final report describes a project in Michigan from 2002-2006 to improve genetic health outcomes of Michigan residents by implementing portions of the state genetics plan. Objectives described include (1) increasing genetic literacy and improving family and health care provider access to information about resources and services for children with special health care needs; (2) expanding methods of early identification of children with special health care needs; and (3) integrating newborn screening with other maternal child health data and service delivery systems to promote linkage with intervention services including the medical home. Report contents include an overview of the project's original goals and objectives, methodology, accomplishments, evaluation, lessons learned, future plans and next steps, and plans for sustainability. Also included are a list of peer reviewed publications and presentations, products developed during the project, and appendices with samples of publications and presentations.
