Annotation: This report describes a Healthy Start program from 1997 to 2002 in Wards 5 and 6 in the District of Columbia to establish an infant mortality reduction program. Report contents include a description of the service initiation; care coordination and case management; service accomplishments; mentoring received from the demonstration project; a description of the consortium; a review of the impact of the program; and other program components such as management and governance, sustainability, local government rule, and the local evaluation. Additional sections of the report describe the fetal and infant mortality review and project data. Attachments to the report include an organizational chart illustrating the Maternal and Family Health Administration in the District of Columbia Department of Health and a chart providing data on services provided. Annotation: This impact report describes the first phase of a Healthy Start project in the Richmond, Virginia area, from 1994-1997 to provide maternal and child health care services to pregnant women, their children and their families. The report describes the implementation of six models during the demonstration phase including case management and care coordination; outreach and client recruitment; education and training; risk prevention and reduction; facilitating services; and adolescent education services. Report contents include service initiative, accomplishments, impacts of systems of care and the community, and other Healthy Start program components. Attachments include a copy of the local evaluation report, the fetal and infant mortality review, aggregate data and major service tables, project area demographic and statistical data tables, and an organizational chart.
